The Student Permit Form (form 34) is used to submit an application for a student permit to study or train at any registered primary, secondary or tertiary level school/institution in Trinidad and Tobago.

If you wish to remain for more than one year to complete your course of study, you will need to submit a Medical Certificate Form 40 and the Physical Examination Form 40A. These forms must be submitted by every applicant 16 years of age and older, or by the parents or guardian on behalf of applicants less than 16 years of age.

NOTE: Students of the University of the West Indies (U.W.I) St. Augustine Campus; Sir Hugh Wooding Law School (U.W.I. St. Augustine); U.W.I Faculty of Medicine, Mount Hope; and St. John Vianney Seminary, Mount St. Benedict (Theology Degree only) do not require Student Permits.

DO NOT use the web browser print icon or print menu. If you do this the document will not print properly. Remember when printing these forms the document must be printed on both sides of the page.
